Clinical Evaluations are a face to face interview and may require more information than the initial session. It is not a counseling session and is tasked with a full review of your alcohol and drug use history including legal history. The evaluator may require additional information which can can be provided back to the office or through an Unum account the evaluator will establish for you.

The Unum application is an electronic record system that will give you 24/7 access to your records involving the clinical evaluation or if required treatment. After releases of information are signed, it makes it easy to share information as necessary.

The minimum fee for a clinical evaluation is $110.00. The evaluation fee is based on state regulations and if there are any special requirements based on your particular situation. The fee will not be more than $155.00.
